About the Role

Provide clinical support and counseling to members and families navigating the emotional realities of dementia.

Responsibilities

• Offer counseling and emotional support to members and caregivers
• Assess needs and connect families with resources
• Collaborate with the care team on member wellbeing

Qualifications

• Active LCSW license in the State of Texas
• Experience in geriatric or dementia-related care preferred

All Journey Together staff are Certified Dementia Practitioners (CDP) and licensed Savvy Caregivers, or are willing to obtain certification.
